We are delighted that your child will be joining us for a Shadow Day at Chesterton Academy of The Holy Family. To ensure a safe and enjoyable visit, we ask that a parent or guardian review and acknowledge the following:

I am the parent/guardian of the student listed on this form and give permission for my child to attend a Shadow Day at Chesterton Academy of The Holy Family.
In the event of a minor need, I authorize school staff to provide basic first aid for my child.
I understand that the school will take every reasonable precaution to ensure my child’s safety during the visit.
